Description: The Notice of Inclusion on a jury roll is the first letter you will receive from the Sheriff's Office in relation to jury service. The notice is to let you know that you are on a jury roll for a period of 12months. During this period you may receive a Summons. At this stage, you can apply to be removed from the jury roll if you meet one or more of the catagories listed on the back of the notice. If you have lost or misplaced the form and wish to make an application to be excused please download this form and mail or fax the form (with appropriate supporting documents) to the Sheriff's Office (Head Office) Locked Bag 5121 Parramatta NSW 2124, fax to 8688 9677.
