Notice of Creditor's Service and Enforcement of Process

Description: This information sheet has been written following change in 2006 to Schedule 2, Civil Procedure Regulation 2005. The change in Regulation has caused a fundamental change in the way the Sheriff's Office will enforce process and charge for enforcement operations. The information sheet is provided to inform creditors/plaintiffs of the changes. To best use the resources of the Sheriff, creditors/plaintiffs are requested to provide any information that may assist us to finalise the process at the first attempt.

However, if you have lost or misplaced the form and wish to make an Application to be excused please download this form and mail or fax it (with appropriate supporting documents) to the Sheriff's Office listed on your summons or call 1300 722 574 (or 9209 8222) for assistance with the correct postal address and/or fax numbers.
Failure to Attend form

Description: If you have failed to attend for jury service you will be issued with a Failure to Attend form. This form gives you an opportuntity to provide a written explanation as to why you do not attend for jury service (Part A). Please include any supporting documents. You may also elect to have the matter heard by a Magistrate at your local court (Part B). Mail or fax it (with appropriate supporting documents) to the Sheriff's Office (Head Office - Sydney) P.O Box A4 Sydney South NSW 1232 or by fax to: 9287 7260 or 9287 7222. For further information with the Failure to Attend form please call 1300 722 574 (or 9209 8222).
